Description:
The ROOTS Permanent Housing program will provide stable housing and case management services to assist chronically homeless individuals with a disability.
Residential: Located in the Franklin Arms apartment building at 3120 W. Wisconsin Avenue, participants are housed in a residential community.
Occupancy: 13 partially furnished apartments are available to house chronically homeless men and women.
Opportunity: The participant has the opportunity to be self-sufficient and productive in their community.
Towards: With guidance and support, program participants work towards self-sufficiency and becoming productive members of the community.
Stability: Stable housing enables the participant the opportunity to establish roots, experience personal growth and sustain self-sufficiency.
