The Viola House, an eight (8) bed CBRF (Community Based Residential Facility). The Viola House is handicap accessible and serves male and female adults with mental illness or developmental disabilities. HCBS Compliant.
A class CNA Nonambulatory CBRF may serve residents who are ambulatory, semi-ambulatory or non-ambulatory but one or more of who not physically or mentally capable of responding to an electronic fire alarm and exiting the facility without help or verbal or physical prompting.
Viola House is a eight (8) bedroom, three (3) bath ranch style one level home with a large kitchen, dining, living, family rooms and laundry facilities. Owned and operated by Assisted Care, Inc., Vernal Nelson, Vice President and Kevin Gross, Manager. The care provider on duty is in charge during the Managers absence from the facility with the Manager being on call for 24 hours.
Eligibility: Mental illness/cognitvie disabilities, ambulatory or non-ambulatory.
Intake procedure: By appointment with administrator.
All ADL's, transportation to appointments, activities, crafts, community events.
Accessibilities: Wheelchairs, walkers.
