Heritage of Elmwood is a non-profit, municipally-owned skilled nursing community with memory care and a very active restorative nursing program that augments the work of licensed therapists. The Board of Directors for Heritage of Elmwood is appointed by the Village of Elmwood Board of Trustees. We are Medicare and Medicaid certified.
