Commonheart Hospice utilizes a team approach to each person’s care, with the patient and family being the primary focus in developing an individualized careplan that best meets their needs and wishes. We begin by providing a free, no obligation evaluation. From there, the Hospice team; consisting of the patient’s physician, Hospice physician, Registered Nurses, Social Workers, Chaplains, Certified Nurse Assistants and Volunteers work closely to assure that everything we do focuses on promoting comfort and meeting the physical, emotional, and spiritual needs of the patient and family.
