Emotionally Disturbed / Mental Illness
Program Services include: Medication Monitoring, Medication Storage, and Medication Record. General Services include: Money Management, Daily Living Skills, Allowances, Information and Reports, Leisure Time Activities, Transportation, Survival Skills, Community Activities, Family Contacts, Supervision, Program Goals.
The clients are mentally ill and developmentally disabled in need of a structured treatment program. Clients must be ambulatory, mentally and physically capable of responding to a fire alarm and exiting the facility without much prompting. No client will be accepted who would require nursing care or constant hospitalization, those who are bedridden, destructive of property or self, unless preventative measures have been taken, those who are physically and mentally abusive to others or any client who have a history of being "fire starters".
All clients shall receive an initial health assessment, which administered not more than 90 days prior to or within 5 working days after admission and annually thereafter. This assessment shall include a tuberculin skin test and documentation of the results at time of admission and annually thereafter. If the test has a positive result, Mystic Creek will initiate its policy and procedure regarding communicable diseases. Annual health assessments will be conducted for other health needs such as dental, optical and other concerns as needed. Mystic Creek will arrange and coordinate these appointments.
