The goal of Stoughton Hospital’s Home Health Services is to provide, safe quality healthcare in a personalized manner in the comfort of your own home.



During home visits, nurses and therapists will educate you and your family regarding how to manage your illness. Staff will address specific problems you are facing, teach skills to speed recovery and reduce the possibility of re-hospitalization, and provide support and education in managing chronic illnesses.



Stoughton Hospital Home Health is state-licensed, Medicare and Medicaid certified and accredited by the Joint Commission. Available Home Health Services include:



Nursing – Registered nurses are on call 24 hours/7 days a week. They provide skilled nursing care for acute and chronic disease conditions, case management of care, medication monitoring, wound care, IV therapy and education regarding chronic disease management. Our nurses will work with your physician to ensure the best possible plan of care for your specific condition.



Home Health Aide – Home health aides provide personal care; assistance with bathing, dressing and toileting; and home therapy programs.



Social Services – Social workers assess and offer resources to meet your social and emotional needs, coordinate community services, and provide counseling and assistance in planning for the future – finding the right resources to meet your needs.



Physical Therapy – Physical therapists provide treatment to improve strength and endurance, as well as increasing functional independence. This may be in the areas of ambulation, transferring in and out of bed, use of assistive devices, instruction in personalized home exercise programs, assessing patient safety, and/or training of family members to safely assist patients to prevent falls or other injuries.



Occupational Therapy – Occupational therapists provide strengthening, coordination and cognitive training to improve arm and hand function and/or increase independence in activities of daily living. This may include self-feeding, dressing, bathing, homemaking, home management, use of adaptive devices or family training to facilitate independence and safety in the home.



Speech Therapy – Speech and language therapists will help you regain or improve speech, language and swallowing skills. They offer training for safe swallowing and improvement of communicationo n skills for those who have or may have cognitive impairments or disabilities.



Trusted Hands Program



Stoughton Hospital’s Trusted Hands Program provides reliable, professional home care and companionship services allowing you or your loved one to remain safely and comfortably at home. Trusted Hands services include:

•Meal preparation

•Errands

•Grocery shopping

•Respite care

•Bathing and personal care
