IRIS – Include, Respect, I Self-Direct – is Wisconsin's Self-Directed Supports Program for older people and adults with disabilities. IRIS builds upon the belief that every person, regardless of age or ability, has the right to lead a meaningful life. You determine what your meaningful life looks like, including where you live, the job you want, the people you spend time with and how you are involved in your community. IRIS recognizes that you have talents, skills, interests, and ideas that deserve attention. With this program, you can customize a plan that meets your care needs and enhances those assets. IRIS is dedicated to helping you create the life you want with the support you need. What is Self-Direction? Self-Direction means that you have increased control over the long-term care goods and services you purchase and how they are provided. You are empowered to make decisions about your life, including how the IRIS program can support your goals. An Independent Consultant can assist participants with self-direction, plan creation and budget management. As of 8/1/2016 there are four consultant agency options:

The participant makes purchases and hires service providers directly or with the help of an employment agency. The participant authorizes payments to providers. The Financial Services Agency pays the bills and does all of the employee-related paperwork including withholding taxes and preparing W-2 forms. To be eligible for IRIS, an individual must need a nursing home level of care as specified by the State and meet the Wisconsin Medicaid eligibility requirements. Milwaukee County residents for more information and to request an assessment for eligibility if you are 18 - 59 years of age please contact the Disability Resource Center at 289-6660. If you are 60 years or older contact the Aging Resource Center of Milwaukee County at 289-6874 .
