Provides a wide range of home medical equipment and supplies to assist patients in increasing mobility, safety, independence and comfort.
Services include: Mobility Aids including wheelchairs, walkers and scooters; Oxygen and Respiratory Therapy services including oxygen, continuous positive airway pressure(CPAP), apnea monitors, sleep studies and ventilators; Patient room and Home Safety Equipment; Nutritional Supplies; and aids for Daily Living.
Home Health Services
The home health team collaborates with physicians to develop an individualized plan of care for each patient. Our goal is to help our patients remain at home whether they are recovering from an illness or injury, dealing with a chronic condition, or healing from surgery.
Your home health care team is led by an RN Case Manager and may also include:
