Grief Support Program is offered to all members of the community. Not just those affiliated with their Hospice and Palliative Programs.

Unity also offers programs throughout the year to anyone who has experienced the death of a loved one.
Unity's Hospice care program is a specialty that provides comfort care to persons who are in the last six months of their lives and are no longer seeking or receiving curative treatment.
Their team includes the patient's physician, nurses, certified nurse assistants, social workers, chaplains, volunteers and grief counselors.
Eligibility: To be medically eligible to receive hospice care, a patient will have a prognosis of 6 months and no longer receiving curative treatment.

Fees: Medicare, Medicaid, private insurance, private pay (sliding scale)
Hours: Mon-Fri 7AM - 5PM Sat-Sun 7AM - 3:30 PM
